This loan agreement is for districts when requesting a high-ticket item. Upon ordering, these terms and conditions create a contract between you and Michigan Department of Education – Low Incidence Outreach (MDE-LIO) (the “Agreement”). Please read the Agreement carefully.
Ownership
You understand that the item listed above are the property of MDE-LIO and that they were purchased using Federal Quota Funds.
You understand that the item are available on loan.
Terms of Loan
The item listed above are available to use as long as the student listed above needs the item to complete academic goals and objectives.
If and when a student no longer needs the item, you agree to return the item to MDE-LIO via UPS. To receive a UPS return label, you agree to contact MDE-LIO at [email protected].
You may transfer the item to another student in need as long as you inform MDE-LIO of the student transfer by contacting them at [email protected].
Repairs
It is understood that an item that needs to be repaired must be sent to MDE-LIO via UPS. To receive a UPS return label, you agree to contact MDE-LIO at [email protected].
You understand that all repairs are paid for using Federal Quota Funds and that the item must be submitted for repair by MDE-LIO. A new item will not be issued during the repair period. Your repaired device will be sent back to you.
