This summer, MDE-LIO will once again provide assistive technology (AT) assessments for students with a visual impairment. These assessments help pair students with AT solutions that meet their needs and enable them to complete tasks as independently as possible.

AT assessments should be part of an ongoing process with the student’s educational team. Assessments are valuable for identifying devices, tools, and services that will help students improve their functional capabilities in achieving individualized education program (IEP) goals.

To request an assessment for your student or child, visit the Blind/Visually Impaired Outreach Support page and follow the steps to complete and submit an Outreach Support Request Form. Please make sure to read the Outreach Support Guidelines on the page before submitting the form.

After receiving the request, MDE-LIO will contact you to set up an appointment for an assessment. Assessments will be conducted August 12-16, and each assessment may take up to three hours.

Documents

Deadline to submit student documents: May 30, 2019

After your appointment is confirmed, please fax or mail the following student documents to MDE-LIO:

  • Current Individualized Education Program (IEP)
  • Class schedule
  • Eye report
  • Completed Learning Media Assessment (LMA)
  • Completed Functional Vision Assessment (FVA)

Assessment Reports

After the assessment is conducted, it may take up to two months to generate and distribute the report. The report will be sent to everyone who signed the Outreach Support Request Form.
